The UPI of health has launched. Here is what actually runs on it.

The Unified Health Interface, or UHI, was launched on 29 June 2026. Most reports called it UPI for healthcare. The idea is simple. You open any health app, search for a doctor, and see doctors from other apps too.

I went through the government's own pages to see what is running three months later. The pages do not agree with each other, and a few things that should be there are missing.

What I found

  1. The launch release, the ABDM UHI page and a later government note on Aarogya Setu 2.0 each describe a different set of live services. Online consultation and in-person visits are where they differ most.
  2. In September the ABDM page listed seven connected apps. Aarogya Setu 2.0, Practo, Apollo 24|7 and Tata 1mg were not among them. The list has since been taken down, so there is now no official list of who is on UHI.
  3. There are still no rules on what an app can charge for a booking, and no published count of bookings.

How UHI works

Today, if you book a doctor on Practo, you can only see Practo's doctors. On Apollo 24|7, you see Apollo's. Each app keeps its own list.

UHI links the apps. Your app sends your search to a government server run by the National Health Authority. The server passes it to every provider app connected to it. Their doctors come back to you in one list. Once you pick a doctor, your app and the doctor's app deal with each other directly.

From closed platforms to one network Today: one app, its own doctors On UHI: any app, any provider Patients Patients Patients App A App B App C Its doctors Its doctors Its doctors Patient app Patient app Patient app NHA gatewaysearch only, no money, no PII Provider app Provider app Provider app Dashed amber line: after discovery, the patient's app and the provider's app talk directly.

Two things are different from a normal booking app. The government server never touches your money. You pay the provider, usually by UPI. And a provider cannot pay to show up first in your results.

UHI is one of three government systems a patient's care can pass through. UHI helps you find the doctor. A consent manager moves your medical records between hospitals when you approve it. The National Health Claims Exchange, which I wrote about in Edition 76, carries the insurance claim.

Here is the full chain for one patient who is admitted for surgery, from the first symptom to the last follow-up, with the part of the system that handles each step and where it stands in September 2026.

Read down the status column. The steps run by the government are live: health IDs, registries, records, scheme claims. The steps that need private doctors and insurers are early. And recovery after the hospital, rehab, physiotherapy and home care, has no digital rail at all.

What is live

There are two official places that list UHI services. One is the Health Ministry's press release from 29 June. The other is the UHI page on the ABDM website, which I checked in the week of 14 September. They should say the same thing. They do not.

Is the service live?

In-person doctor visitLiveComing soon

Shaded rows disagree. Lab tests and vaccination are not live on either. Pharmacy is "next phase" in the release and missing from the website.

A separate government note on Aarogya Setu 2.0 adds a third version. It says the app books both online and in-person consultations through UHI. The launch release has an error of its own too: it says four services are live and then lists five.

None of this means UHI is broken. It means the public record has not caught up with the product, and three official sources still give three answers.

Look at the three services both sources agree on: blood banks, PMJAY hospitals and ambulances. The government already runs or funds all three. The two that need private doctors to sign up are the two the sources cannot agree on.

Who has joined

In the week of 14 September, the ABDM page listed seven apps connected to UHI. When I checked again, the list was gone and the page said "Current Network Participants: Coming Soon". The table below is the September list, the last one published.

Patient appsProvider appsNot on the September list
eClinicalWorkse-RaktKosh (blood banks)Aarogya Setu 2.0
DRiefcaseMediBuddyPracto
Aarogya1Bajaj Finserv HealthApollo 24|7
Pristyn CareTata 1mg

MediBuddy and Bajaj Finserv Health have real doctor networks, so private doctors are on UHI. I did not expect that this early.

Aarogya Setu 2.0 was missing, even though the government says it books through UHI. That may only mean different pages are updated at different times. It matters anyway. Developers, hospitals and patients need a participant list they can trust before they treat UHI as public infrastructure, and right now there is no list at all.

The large doctor apps were not on the list either. For Practo or Apollo 24|7, the list of doctors is the business. Joining UHI means their doctors show up inside other companies' apps. So far nothing tells them what they get back.

Is it UPI yet?

UPI had a few things in place from the first day. UHI does not have them yet.

UPIUHI today
Moves the moneyYesNo
A regulator that can make everyone joinYes, RBI for banksNo, not for private clinics or apps
Rules on feesYes, from launchNo, marked "coming soon"
Usage numbersPublished every monthNone published

If any health system deserves the UPI comparison, it is the claims exchange from Edition 76. Two regulated parties, one central switch, one standard message. UHI is closer to ONDC, the government's open network for online shopping. ONDC also spent years trying to get the big sellers to join.

Edition 76 found that the claims exchange works where the government pays and stalls where it does not. UHI shows the same thing. The services that work are the ones the government already runs.

What to watch

  • NHA publishes the fee rules for apps on UHI.
  • NHA publishes a participant list and keeps it up to date, with Aarogya Setu 2.0 on it, and one of Practo, Apollo 24|7 or Tata 1mg joins.
  • NHA publishes how many bookings UHI handles each month.

If two of these happen by March 2027, UHI is turning into a market. If none does, it stays a useful directory of government health services.
